Voluntary Vision Plan

OASD is now partnering with Vision Benefits of America (VBA) to offer employees and dependents voluntary vision insurance.

Eligibility:

Any Oxford employee regularly working 15 hours weekly or more.

Enrollment: (Due by September 11, 2023)

Please review the two plan designs linked below. (Base or Buy-Up). Select the corresponding enrollment form and save a copy. Please fill out all requested information and electronically sign the application at the bottom. Send all completed applications to Evette Shaffer @ Eshaffer@oxfordasd.org. Please note, the application must be sent by September 11, 2023 for the 23-24SY. Coverage will begin in September 2023.
