Welcome Prospective NHS Members!
We are glad you have taken an interest in The Old Oak Chapter of the National Honor Society.
Requirements to join National Honor Society include, but are not limited to:
- Have a cumulative weighted GPA of at least 3.5 at the end of your sophomore or junior year.
- Demonstrate active involvement in school and community activities.
- Demonstrate commitment to service in the school and community.
- Demonstrate leadership abilities.
- Be of upstanding character.
How to Become an NHS Member (for Students, for Parents)
Oxford Old Oak Chapter Application Process
Students with a cumulative weighted GPA of at least 3.5 at the end of their sophomore or junior year will be invited to join NHS at the beginning of their junior or senior year. If you have received an invitation to join National Honor Society, then you will need to submit the forms below to verify your candidacy. The forms have been made available as both a Microsoft Word document and as a PDF for your convenience.
Invitations for eligible students will be sent in early September each year. Candidates receive the Candidacy Acceptance Form (Word) (PDF), which must be completed and turned in to an advisor to indicate intent to apply, receive an application identification number, and begin the application process.
The Candidate Evaluation Rubric (PDF) is used to determine eligibility for membership into the Old Oak Chapter of the National Honor Society.
Application Forms:
Scan into one pdf document with pages upright and submit electronically to your ID folder in the NHS Sharepoint: *Instructions for scanning documents to PDF and signing documents on an iPad or iPhone
- Directions and Student Checklist (Word) (PDF) - Used to keep track of paperwork that is completed.
- Student Information Form (Word) (PDF) - Completed by candidate to verify school and community activities.
Submitted in hardcopy to Mrs. Liljestrand or Mrs. Warren
- Service Recommendation Letter (Word) (PDF) - One (1) service recommendation letter is needed for the packet.
- Leadership Recommendation Letter (Word) (PDF) - One (1) Leadership Recommendation letter is needed for the packet.
Submitted Online by Teacher (you must email them the link)
- Teacher Recommendation Form - Two (2) Teacher Recommendations are needed for the packet (share link with teachers).
The NHS Application is due by 3pm on October 11, 2024. No late submissions will be accepted.
- Prospective members will be notified of their acceptance by early November.
- The Induction Ceremony will take place November 20, 2024 at 6 pm, with a rehearsal on November 19 at 2:30 pm.
